PATIENT NAVIGATOR PROGRAM

Creating Change with Culturally Conscious Care

Determi-Nation is partnering with medical schools to institute a patient navigation care model across rheumatology and dermatology for people of color. This program started with SUNY Downstate Health Sciences University in Brooklyn in Fall 2022.

How the Care Model Works

This care model offers at-risk patients of color with Psoriasis (PsO) and Psoriatic Arthritis (PsA) hands-on support in navigating the healthcare system, provided by first- and second-year medical students. These student navigators assist with tasks such as language translation, appointment scheduling, and medication refill reminders, ensuring patients can effectively manage their conditions. In exchange, students gain essential experience in providing culturally conscious care, preparing them to become more effective physicians.
